RESPIRATION AND THE AIRWAY Intraoperative positive end-expiratory pressure evaluation using the intratidal compliance-volume profile

چکیده
Editor’s key points † The use of PEEP is beneficial in mechanically ventilated critically ill patients with acute lung injury. † PEEP may also be useful during surgery under general anaesthesia, but the optimum levels are uncertain. † In this small observational study, a PEEP setting of 5 cm H2O was insufficient to prevent derecruitment and a reduction in compliance. † Higher, individualized settings of PEEP are probably required for most patients, but prospective studies using a variety of PEEP settings are needed. Background. Lung-protective mechanical ventilation during general surgery including the application of PEEP can reduce postoperative pulmonary complications. In a prospective clinical observation study, we evaluated volume-dependent respiratory system compliance in adult patients undergoing ear–nose–throat surgery with ventilation settings chosen empirically by the attending anaesthetist.

The shape of intratidal resistance-volume and compliance-volume curves in mechanical ventilation - an animal study.
In respiratory system mechanics, the shape of the intratidal pulmonary compliance-volume curve can be used to detect atelectasis or overdistension in the mechanically ventilated lung and thus to optimise the ventilator setting in terms of positive end-expiratory pressure (PEEP) and tidal volume (VT). To this end, a set of shapecategories had been suggested. Furthermore, a characteristic behavio...
متن کاملConventional mechanical ventilation
INTRODUCTION — Conventional mechanical ventilation refers to the delivery of full or partial ventilatory support by a volume-cycled mechanical ventilator or by pressure support. It also can include the maintenance of positive airway pressure at the end of exhalation, ie, positive end-expiratory pressure (PEEP) [1]. (See "Physiologic and pathophysiologic consequences of positive pressure ventila...
متن کاملPersonalizing mechanical ventilation according to physiologic parameters to stabilize alveoli and minimize ventilator induced lung injury (VILI)
It has been shown that mechanical ventilation in patients with, or at high-risk for, the development of acute respiratory distress syndrome (ARDS) can be a double-edged sword. If the mechanical breath is improperly set, it can amplify the lung injury associated with ARDS, causing a secondary ventilator-induced lung injury (VILI). Conversely, the mechanical breath can be adjusted to minimize VIL...
متن کاملNon-invasive determination of alveolar pressure during mechanical ventilation.
The development of inadvertent positive end-expiratory pressure (PEEPi) in ventilated infants is of clinical relevance and difficult to measure non-invasively. A method for estimating end-expiratory alveolar pressure by applying a multiple regression analysis to airway opening pressure, flow and volume recordings during mechanical ventilation was evaluated. In eight open-chested, paralysed and ...
